Blue Essentials™
Overview
Blue Essentials™ plans for three to 19 employees offer a flexible rates and customizable plan options, allowing you to design the benefits package that best suits your business.
Basic coverage
Accidental dental services
- Unlimited coverage
Aerochambers/nebulizers/C-PAP accessories
- Charges for the purchase of aerochambers, nebulizers and Continuous Positive Airway Pressure accessories (including masks, tubing, etc.) when prescribed by a Physician
Ambulance services
- Air and ground
- Unlimited coverage
Blood pressure monitors
- Charges for the purchase or rental of a blood pressure monitor on the written authorization of a Physician
- A limitation of the purchase of one blood pressure mobitor per Participant every five calendar years is allowed
Cardiac rehabilitation
- Charges for treatment rendered to cardiac patients under a recognized cardiac rehabilitation program where such treatments have been prescribed by the attending Physician for:
- Rehabilitation after myocardial infarction, coronary bypass surgery or valve replacement; or
- The management of angina pectoris or other diagnosed cardiac disease
- Payment is limited to a lifetime maximum of $300 per Participant
Dental care
Maximums of $750, $1,000 or $1,500 per person per calendar year. Choose from the following dental services:
- Basic dental services: cleanings, checkups, extractions, fillings, denture repair, root canals and gum treatment
- Major dental services: bridges, crowns and dentures
- Orthodontic services: prevention or correction of irregularities of the natural teeth for eligible dependent children
Diabetic equipment & supplies
- Unlimited coverage
Eye examinations
- Maximum $125 per person in two calendar years (every calendar year for dependent children)
Hearing aids
- Maximum $500 per person in three calendar years
Hospital services
- Private/semi-private accommodation
- Unlimited coverage
Medical equipment
- Wheelchairs, manual hospital beds, oxygen equipment rental
- Unlimited coverage
Mobility aids
- Charges for the purchase of bathroom rails, bath seats, raised toilet seats or reachers, on the written authorization of a Physician

Out-of-province referral
- Maximum $50,000 per treatment
- Lifetime maximum $100,000 per person
Out-of-province emergency services & travel assistance
- 100% reimbursement for business or vacation travel
- Up to $5,000,000 in emergency medical benefits
- No deductible
- Option to choose a maximum of 60 or 90 days per trip
- Unlimited number of trips per year
- Travel Assistance available 24 hours a day, 365 days a year, toll free in North America, by collect call elsewhere
Prescription drugs
- Maximums of $1,000, $2,500 or unlimited per person per calendar year for prescribed drugs, including Saskatchewan Formulary and non-formulary drugs
- Includes convenient Pay Direct Drug Card
Private duty nursing services
- Maximum $10,000 per person in three calendar years
Prosthetic & medical appliances
- Unlimited coverage (except wigs – maximum $500 per person per calendar year)
Registered paramedical/medical practitioners
- Maximum $350 or $500 per person per calendar year for each practitioner (chiropractor, chiropodist/podiatrist, physiotherapist, massage therapist, speech therapist, clinical psychologist/social worker/counselor, osteopath, naturopath, acupuncturist, audiologist, and dietician)
Second Opinion® service
- Confirmation of diagnosis and treatment for a serious medical condition
24-month survivor benefits
- Covers spouse and dependents for up to 24 months in the event of the employee’s death
Vision care
- Maximums of $200 or $300 every two calendar years (every calendar year for dependent children) for eye glasses, contact lenses, visual training and remedial eye exercises, and laser eye surgery

Life Plan Benefits*
- Choose coverage of $10,000, $25,000 or $50,000 or customize to meet the needs of your organization. The amount applies to all life plan benefits.

Accidental death & dismemberment*
- The Accidental Death & Dismemberment benefit is included in the total maximum Life plan benefits
- Lump-sum payment in the amount of coverage chosen to the named beneficiary in the event of death by accident
Enhanced critical illness insurance*
- The Critical Illness benefit is included in the total maximum Life plan benefits
- Lump-sum payment in the amount of coverage chosen to help cope with a severe critical illness or disease
- 25 eligible conditions, including Cancer (Life Threatening), Aortic Surgery, Coronary Artery Bypass Surgery, Heart Attack, Heart Valve Replacement, Blindness, Severe Burns, Deafness, Loss of Limbs, Loss of Speech, Occupational HIV, Aplastic Anemia, Bacterial Meningitis, Benign Brain Tumour, Coma, Dementia including Alzheimer’s Disease, Kidney Failure, Loss of Independent Existence, Major Organ Failure on Waiting List, Major Organ Transplant, Motor Neuron Disease, Multiple Sclerosis, Paralysis, Parkinson’s Disease, and Stroke.
- 4 non-threatening conditions are eligible for a partial benefit payment including Coronary Angioplasty, Ductal Carcinoma In Situ Of The Breast, Stage 1A Malignant Melanoma, and Stage A (T1a or T1b) Prostate Cancer.
- 7 childhood conditions are eligible for a full benefit payment including Cerebral Palsy, Congenital Heart Disease, Cystic Fibrosis, Muscular Dystrophy, Type 1 Diabetes Mellitus, Autism, and Down Syndrome.
- $10,000 employee/$2,000 spouse/$1,000 child OR
$25,000 employee/$5,000 spouse/$2,500 child OR
$50,000 employee/$10,000 spouse/$5,000 child
Group and Dependent life insurance*
- The Group and Dependent life insurance benefits are included in the total maximum Life plan benefits.
Life Insurance
- Lump-sum payment in the amount of coverage chosen to the named beneficiary in the event of death
Dependent Life Insurance
- $5,000 spouse/$2,500 child OR
$10,000 spouse/$5,000 child
Employee and Family Assistance Program (EFAP)
- Access to counselling services provided through Homewood Human Solutions
- No pre-set limit to the number of assessed conditions (cases) per plan member
- Counselling services are provided within the parameters of a short-term model focused on problem-solving and finding solutions
- Services are offered in person, via phone, or online
- Access to online resources including e-learnings, articles, and health and wellness assessments
Online Doctors by Maple
- Connect plan members and their families with Canadian-licensed general practitioners in under five minutes

Weekly indemnity and Long term disability*
Weekly Indemnity
- Weekly indemnity insurance provides weekly benefits for up to 17 weeks at a benefit of $638 per week.
Long Term Disability
- Long term disability insurance provides monthly benefits for disabilities lasting four months or longer at a benefit of $2,000 per month.
